Reach Out

Fill out the contact form and someone from Dr. Menousek's team will call you to chat, answer your questions, and schedule your child's evaluation. Typical wait time is 4-8 weeks. 

Create Your Portal Account

You'll receive a link to a secure patient portal where you'll create an account with your information, your child's information, and a secure way to upload your documents. If uploading documents is a challenge, send us a message or give us a call, and we can help. Insurance information is required before your appointment. If you don't have your physical card, you can find your member ID by calling your insurance provider or logging into your insurance member portal. See a list of insurances we accept. 

If you have any of the following, please upload them or bring them to your appointment:

  • Previous evaluation reports

  • School IEP or 504 plans

The Evaluation

The evaluation is a single two-hour in-person appointment. Dr. Menousek's office is relaxed and kid-friendly; there are toys, it's casual, and your child won't feel like they're being tested. Dr. Menousek will observe and interact with your child directly. After the appointment, Dr. Menousek may send rating scale questionnaires to you and your child's teacher to gather additional input before finalizing the results. Feel free to bring snacks and a favorite toy.

About a week before your appointment, you'll receive a text message with a reminder, a Google Maps link, and specific arrival instructions. Dr. Menousek's office is located inside Modern Works, which is a converted grocery store building, so it can look a little unexpected from the outside. The text will make sure you know exactly where to go.

The Report

Before you leave, you'll schedule a follow-up telehealth call. Typically, this virtual visit lasts about 45 minutes. Dr. Menousek walks you through the full report, explains what the results mean for your child, and answers all your questions. Most families receive their results within a week of the evaluation. Your child does not need to be present for the results call.

The written report provides an official clinical diagnosis, this not the same as a school assessment. This means it's accepted by your child's school for accommodations and IEP support, by insurance providers, and by any medical or specialist team involved in your child's care.
